Becoming a CBMT Approved Provider

Applying for CBMT Approved Provider status is easy. The first step in pursuing CBMT Approved Provider (AP) status is submission of the CBMT Approved Provider Two-Year Provisional Application. Upon approval of the application by CBMT, you will enter two years of provisional Approved Provider status.

Step 1: Read the Approved Provider Manual and watch this video tutorial:

The Approved Provider Manual contains comprehensive information on applying for and serving as an Approved Provider for continuing music therapy education coursework.

Step 2: Submit your Application & Forms

Follow the steps in the Approved Provider Manual and video tutorial to access your online account to complete the Approved Provider Application.

Standardized CMTE Opportunity Form

The following forms can be completed and uploaded into the Approved Provider Application.

         How to Write Measurable Objectives – please review these examples prior to submitting objectives for review.

You may utilize the third page as many times as necessary for multiple presenters.

This form helps the presenter, the Approved Provider and CBMT evaluate the quality of each course.

Please note that CBMT requires participants to include their certification number.

CMTE Certificate Template 

Opportunity Certificate Template: We encourage Approved Providers to personalize certificates with their own logos, while maintaining required content. The two statements at the bottom of the template are particularly important.

Maintaining Provider Status:

During your provider status you can submit additional opportunities at any time.  All opportunities are approved for a one-year period and require renewal each year. Below is a video on how to submit additional opportunities in your CBMT account.

At the end of the two-year provisional cycle, you may apply for five-year status. You will be audited as part of the process, and upon successful completion of the audit and the CBMT Approved Five-Year Application, you will be granted five-year Approved Provider status.

Continuation of five-year Approved Provider status requires timely payment of all fees, submission of all required materials, successful participation in the audit when selected, and completion of the five-year application when due.
