Instructions for Completing the Electronic Summary of CMTE Activites Form

• Use the link below to access the Summary of CMTE Activities form. 
• In order to qualify for CMTE credit, activities reported must relate to at least one area of the CBMT Scope of Practice. 
• Report CMTE credits using whole numbers only. Round down to nearest whole number. Convert CEU's and clock hours to CMTE credits. (see Recertification Manual.)
• Enter each activity under the appropriate category (abbreviate where possible). Refer to Manual for clarification.
• Report all CMTE credits earned during each calendar year. While 100 CMTE credits are required per 5-year cycle for recertification, there is no limit on the number of CMTE credits that may be reported.
• DO NOT SUBMIT DOCUMENTATION UNLESS REQUESTED TO DO SO FOR AUDIT.
